We are here to help facilitate the transition of individuals with disabilities from nursing homes and other institutions to home and community-based residences, as well as providing assistance to individuals with significant disabilities who are at risk of entering institutions so that the individuals can remain in the community. TLCIL also helps facilitate the transition of youths with significant disabilities who have completed their secondary education or otherwise left school, to post-secondary life.
